SECTION III: RESPONSIBILITIES
Your Responsibilities as an Accredited PAIRS Provider:
1. Teach a minimum of four PAIRS classes per year to a minimum total of 32 participants.
2. Inform the Foundation promptly about your professional activities pursuant to this Agreement and your use of the PAIRS trademarks and copyrighted materials.
3. Inform the Foundation immediately upon cancellation or termination of a scheduled program by email to info@pairs.com.
4. Ensure the PAIRS’ trademarks are not used as part of your business, website or professional name without prior written permission.
5. Prior to use, distribution or publication, forward to the Foundation copies of all booklets, pamphlets, brochures, videos, PowerPoint or other substantive produced materials developed by using any reference to the PAIRS’ trademarks or the use of PAIRS copyrighted materials. Unless otherwise agreed, such materials become the property of the PAIRS Foundation.
6. Pay a participant fee to the Foundation for each individual you accept into a PAIRS program, class, seminar, workshop or retreat in compliance with the requirements detailed in this agreement. No fee is due to the Foundation for your sharing of PAIRS concepts and skills in your individual work as a licensed member of the clergy or behavioral health professional when conducted in private with an individual, couple or family. Unless otherwise agreed, fees must be paid to the Foundation at the time materials are ordered.
7. Send to the Foundation a Year End Report of PAIRS Activities by October 1 of each year.
8. Send to the Foundation an annual License Renewal Form and Payment at rates indicated on the current PAIRS Annual Licensing Form.
9. Obtain signed Participant Agreement (Informed Consent) forms from each participant prior to or at the start of any PAIRS program and send either a scanned image of the signed Participant Agreement or original forms promptly after the start of a PAIRS program.
10. Submit electronic rosters on forms provided by the Foundation promptly after the start of a PAIRS program and End-of-Class Evaluations and Self-Assessments promptly following the completion of the PAIRS program. Forms can be sent by courier, registered mail, electronically to info@pairs.com, or by fax to (954) 206-0281.
